Peri-implantitis (Prof. Dommisch, PD Dr. Jepsen)
It is well known that both periodontitis and peri-implantitis are caused by bacterial infections, but fundamental differences exist in host-microbe interactions with oral mucosal tissues as well as the variety of local and systemic risk factors.
Peri-implantitis is a challenging condition where the treatment choice should be based on the severity of the disease and the configuration of the peri-implant defect. Resective approaches have been shown to be the treatment choice for horizontal and/or wide vertical bone defects. There are several protocols to improve the predictability of these resective approaches, such as performing implantoplasty, using systemic antibiotics or modifying the quality of peri-implant soft tissues. In addition, the role of supportive therapy plays a role in the long term.
Techniques and goals for preventing peri-implantitis by maintaining a keratinised mucosa around the implant are discussed. We will provide insights into regenerative and resective approaches to the treatment of peri-implant disease. This course will illustrate what to consider when managing implant complications, what surgical approaches are available and how to predictably promote infrabony regeneration of peri-implant defects. Specifically, our own surgical protocol for strategic therapy will be presented.
